Sacred Heart Family of Faith Registration Form

Welcome to our Family of Faith Religious Education program at Sacred Heart Catholic Church for children in grades Pre-K to Grade 12 and Sacramental Preparation.
The focus of the program is to support YOU, the parent, as your child's primary educator. Using this approach, will help the family grow in their faith together and continue to gather as community in Christ. This format allows you to incorporate lessons into your everyday life as a Catholic family and will help you in forming young disciples who are moved to serve God and His Church.
Parents will be required to attend one session a month with their child/children.
Materials will be delivered each month to all parents and students. Sacramental preparation will have additional sessions for the students. Schedule of sessions will be available prior to the first meeting date.

NOTE: If your child was not baptized at Sacred Heart, you must provide a baptismal certificate at the time of registration.
